What is ear pain?

Ear pain, medically called otalgia, is pain or discomfort felt in or around the ear. It may start inside the ear itself. Sometimes the actual source is nearby, such as the jaw, teeth or throat. The feeling is not the same for everyone. It may be sharp, dull, throbbing or burning. Some people mainly notice pressure, fullness or a blocked-ear feeling.

Ear pain itself is a symptom, not a single disease, and causes can range from ear infection and wax buildup to pressure problems or referred pain. For this reason, proper ENT evaluation is important to find the actual cause before choosing the right ear pain treatment.

Types of ear pain we treat

Ear pain can start from different parts of the ear, and sometimes the actual cause is outside the ear. We check the ear canal, eardrum and related symptoms before planning treatment. Our otology care in Dhaka covers common earache, blocked-ear pain, pressure problems and referred otalgia.

Ear infection pain thumb

Ear infection pain (Otitis-related otalgia)

Outer or middle ear infections can cause throbbing pain, pressure, fever, discharge or reduced hearing. Treatment depends on the type of otitis and what we find during the ENT examination.

Impacted earwax pain thumb

Impacted earwax pain (Cerumen impaction)

Hard or tightly packed earwax can press inside the ear canal and cause pain, fullness or muffled hearing. We assess the blockage first and remove impacted cerumen safely when it is needed.

Ear pressure and blockage thumb

Ear pressure and blockage (Aural fullness)

Eustachian tube problems, a recent cold or pressure changes can make the ear feel blocked or heavy. Popping sounds, muffled hearing and ongoing pressure may occur at the same time.

Eardrum pain thumb

Eardrum pain (Tympanic membrane-related otalgia)

Inflammation, injury or perforation of the eardrum can cause sudden or ongoing ear pain. It may also come with discharge, hearing changes or sensitivity to pressure, so careful examination matters.

Foreign body in the ear thumb

Foreign body in the ear (Aural foreign body)

Small objects trapped in the ear canal can cause irritation, swelling, blockage and earache, especially in children. We remove the object carefully to reduce the risk of canal injury or damage to the eardrum.

Referred ear pain thumb

Referred ear pain (Secondary otalgia)

Sometimes the ear itself looks normal, but the pain comes from the jaw, teeth, throat or nearby structures. We check for these secondary causes so the treatment focuses on the real source of the pain.

Common ear pain symptoms

Ear pain may come alone or with other symptoms such as blocked hearing, pressure, discharge or dizziness. The pattern of these symptoms often helps the ENT specialist understand what may be causing the earache.

  • Sharp pain inside the ear
  • Dull or constant earache
  • Throbbing ear pain
  • Burning or irritated feeling in the ear
  • Ear pressure or fullness
  • Blocked-ear sensation
  • Muffled or reduced hearing
  • Fluid, pus or discharge from the ear
  • Ringing or buzzing in the ear
  • Itching inside the ear canal
  • Fever with ear discomfort
  • Pain while chewing or moving the jaw
  • Ear pain when swallowing

Ear pain treatment for children and adults

Ear pain can affect both children and adults, but the common causes and symptoms may differ by age. We assess the ear, hearing changes and related symptoms first, then plan treatment around the actual cause.

Children

Children may develop ear pain from infection, middle ear fluid, impacted wax or a foreign body in the ear canal. We use a careful ENT examination and check for fever, irritability, hearing changes or repeated earache before treatment.

  • Gentle ear examination
  • Check for middle-ear infection or fluid
  • Check for wax or foreign objects
  • Medication when clinically required
  • Hearing assessment where necessary
  • Follow-up for repeated ear pain

Adults

Adults may have ear pain from infection, earwax blockage, Eustachian tube problems, eardrum issues or referred pain. We also check the jaw, teeth and throat when the ear itself does not fully explain the discomfort.

  • Ear canal and eardrum examination
  • Check for infection, wax and pressure problems
  • Assessment of sinus/allergy-related symptoms
  • Evaluation of jaw, dental or throat-related referred pain
  • Hearing test when hearing is affected
  • Treatment based on the diagnosed cause

Ear pain diagnosis process we follow

We begin with the ear itself, then look at the wider picture. At Prof. Dr. Kamrul Hassan Tarafder, the assessment covers how the pain started, how long it has been there and what other symptoms appear with it.

  • Symptom and Medical History Review: We ask about the type of earache, hearing changes, discharge, fever, recent cold, swimming, air travel or previous ear problems.
  • Ear Examination (Otoscopy): We examine the ear canal and eardrum for infection, swelling, impacted wax, fluid, discharge or perforation.
  • Hearing Assessment: We check hearing when ear pain comes with muffled hearing, reduced hearing or a persistent blocked-ear sensation.
  • Tympanometry When Needed: We may use tympanometry to assess middle ear pressure, Eustachian tube function or fluid behind the eardrum.
  • Jaw, Mouth and Throat Examination: We check the jaw, teeth, mouth and throat when the ear looks normal but referred ear pain is still suspected.
  • Diagnosis and Treatment Planning: After finding the likely cause of the otalgia, we plan treatment based on the ear condition, symptoms and examination findings.

When to see an ENT specialist for ear pain?

A mild earache may improve on its own. But pain that keeps coming back, becomes stronger, or appears with hearing changes or discharge needs proper assessment.

  • Ear pain lasts more than a few days or keeps returning.
  • The pain becomes severe or gradually gets worse.
  • Fluid, pus or blood comes from the ear.
  • Hearing becomes muffled, reduced or suddenly changes.
  • The ear stays blocked or under pressure for a long time.
  • Fever occurs with significant ear pain or discharge.
  • Dizziness, vertigo or balance problems appear with the earache.
  • Swelling or redness develops around or behind the ear.
  • A foreign object becomes stuck inside the ear canal.
  • A child has repeated ear pain, irritability or noticeable hearing difficulty.
  • Ear pain continues even when the ear looks normal, as jaw, dental or throat problems may cause referred otalgia.

  2. Repeated ear infections
  3. Persistent ear discharge
  4. Eardrum damage
  5. Hearing problems
  6. Chronic middle ear problems
  7. Spread of infection

FAQs about ear pain treatment in Bangladesh

Ear pain can come from infection, wax, pressure, an eardrum problem or even somewhere outside the ear. These common questions explain what may cause earache, how ENT doctors check it and when treatment becomes necessary.

What are the common causes of ear pain?

Common causes include middle or outer ear infection, impacted earwax, Eustachian tube dysfunction, pressure changes, eardrum injury and a foreign body in the ear. Sometimes the pain comes from the jaw, teeth or throat instead.

Can ear pain happen without an ear infection?

Yes. Not every earache means infection. Wax blockage, air-pressure changes, TMJ problems, dental disease and throat conditions can all cause otalgia, even when the ear itself looks normal.

Can impacted earwax cause ear pain?

Yes. Hard or impacted earwax can cause pain, pressure, blocked hearing or a feeling of fullness inside the ear. Avoid pushing cotton buds or other objects into the ear, as this can push the wax deeper.

Why does my ear hurt when I swallow?

Pain during swallowing may come from the throat rather than the ear itself. Sore throat, tonsillitis and other throat inflammation can send referred pain to the ear through shared nerve pathways.

Can jaw or dental problems cause ear pain?

Yes. Temporomandibular joint problems and dental infections are common causes of secondary or referred ear pain. You may notice the pain more while chewing, opening the mouth or when you also have tooth or jaw discomfort.

How does an ENT specialist diagnose ear pain?

The ENT specialist reviews the symptoms and examines the ear canal and eardrum for infection, wax, fluid, swelling or injury. Hearing tests, tympanometry, or examination of the jaw, mouth and throat may also be needed when the cause is not clear. Prof. Dr. Kamrul Hassan Tarafder follows a diagnosis-based approach for otology problems at his Dhaka chambers.
